Precursor: I was fighting Artoz around WB, got him low, chased him into WB, I got out with 11 hps, then again a couple tics later with 6 hps so I went north to roots fade when Artoz and Tibegex hit me roots, I got out of there with 15 hps, then went to slate where this happened.
I definitely felt bad about this, it was pretty clear he didn't realize the door was there or what the name was.
All the same, was some intense escapes, thought I was for sure dead several times.

Just north of Crevice, south of Hill zone. Small cave system with some bats in it and that pat - it roams but I leave slate open and trap the pat in there whenever I can.
