Incapped and died while not being touched. I was hit with a tremendous fireball before (which took me to crit), perhaps there is residual 'bleed' dmg from fireball I didn't know about?

It looks like Bianca weaved you again, which didn't show a message because it did enough dmg to incap you. Happens a lot. I'm possibly answering a question that wasn't asked.

Never shows in this situation as far as I'm aware.

yeah, weave message shows after dmg. If you are incapped by the damage, then you do not see the message. Kinda like when you get blicked you don't get to see who did it.
