It has been some time since your last visit. Your annual check up is now overdue. You are scheduled at the Sidona office for an evaluation of your health. If you are unable to make the trip to Sidona, you may reschedule at the Whitebridge location, or contact another Wisdom at the location nearest you.
Please bring one form of identification, any vaccination records, and a list of current potions you have been prescribed to take. All efforts to provide refills of those prescriptions will be made at the time of your appointment.
Remember, if you are experiencing any life threatening symptoms such as shortness of breath, numbness or tingling, irregular heartbeat, disorientation, puncture wounds, crushed skull, broken bones, fractures, unusual swelling or rashes, DO NOT WAIT for this appointment and seek immediate help from your nearest Wisdom.
