Two great wooden pillars stand along the northwestern edge of the parapet,
supporting bits of an upper wall that has yet to fall or be torn down. A
narrow passageway spreads to the southwards into the keep bordered by two
piles of jagged rubble, mixed within are bits and pieces of yellowed bone
fragments, compiled of the ancient walls and roof that once bound the top
dome of this long ruined mountain fortress. A tattered sign hangs on the
wall, with a crudely written message.

I guess possibly it might not be a bug, and that someone just accidentally gave it an extra one, but that seems very unlikely and it just did not get properly consumed in the making of the razor longsword.

Not a bug, razor sword doesn't use a ruby in the recipe so it wasn't consumed the first time you gave it to the apprentice.
